Notice Regarding Availability of IRS Form 1095
In accordance with applicable federal reporting requirements, Form 1095 (Health Coverage Statement) is available upon request. This form is not required to be filed when submitting individual tax filings. It is informational to verify that you had affordable employer-sponsored health coverage that met ACA requirements. However, employees or former employees who would like to receive a copy may request one by contacting the Human Resources Department at djhrteam@devaughnjames.com, or via phone 316-888-8888. Once requested, the form will be provided to you promptly in accordance with applicable regulations. If you have any questions regarding this notice or your Form 1095, please contact Human Resources.
